Do you have a compliment?

You have a compliment here is how you can express it:

  1. Speak directly to the individual involved.
  2. Speak directly to the Program Manager.
  3. Use the online/printable forms provided below.


Do you have a concern?

If you have a concern, here’s how to address it:

  1. Talk directly to the person involved.
  2. If that’s not comfortable or if the issue persists, talk to the Program Manager.
  3. If the problem continues or if you prefer a formal approach, find online/printable forms below or ask CMHA staff for a paper copy.
  4. To submit a formal concern verbally, call 705-726-5033 and ask for the Quality and Risk Management Lead. They will inform the CEO of all complaints.

When filing a formal concern:

  • The Program Manager will try to resolve it within 5-10 business days.
  • If you asked to be contacted, expect a response within 5-10 business days.
  • Unresolved concerns will be escalated to a Program Director within 5-10 business days.
  • If still not resolved, it goes to the CEO, who decides within 15 business days.
  • All concerns will be resolved within 35 business days.
